Surgical Treatment of Lumbar Spinal Stenosis with Wiotse Instrumentation

Abstract
To evaluate the effects of the wide laminectomy and posterolateral fusion using transpedicular screws and malleable rods, bone graft for surgical treatment of spinal stenosis, the authors reviewed 81 cases of patients from 1991 to 1993 who were
treated
at Inje University Paik Hospital and followed more than 12 months.
Of the 81 patients, 33 patients were degenerative spinal stenosis, 18 patients were spondylolisthetic spinal stenosis, 28 patients were combined spinal stenosis and 2 patients were iatrogenic spinal stenosis.
One level involvement was 45 cases(65.6%), two level, 28 cases(34.6%), three level, 8 cases (8.6%) and four in l case(1.2%).
75 patients(92.5%) demonstrated successful fusion after their initial precedure, two pedicle screws showed loosening. No rods migrated or was broken.
According to Kirkaldy-Willis' criteria, the postoperative results were as follows; Excellent in 44 cases (54.4%), Good in 32 cases (39.5%), Fair in 4 cases (4.9%) and Poor in l cases(1.2%).
In conclusion, Wiltse instrumentution is a clinically and radiologically reliable method of stablization after extensive posterior decompression.
